Referring a Patient for Inpatient Withdrawal Management

While a referral from a primary care provider is preferred, patients may self-refer. All patients entering the Withdrawal Management Service must be willing and able to give consent. The Withdrawal Management Service at Archbold Grady accepts pregnant and post-partum mothers.

Primary care providers should follow the following procedure to refer a patient for inpatient withdrawal management.

Step 1 - Call Our Admissions Team

Call our admissions team at 229-516-3494. If directed, please leave a message with your name and number. We will promptly return your call.

During this call, you will provide the patient's demographic information, a description of their symptoms and substance history, and their current medical status.

Step 2 - Send Documentation

Please fax the following documentation, if available, to 229-584-7128.

  • Notes documenting signs and symptoms
  • Medication Administration Record (or current medication list, if no MAR)
  • Admission Face Sheet, Insurance Information, Demographics
  • Psychiatric assessment and any psychiatric history or documentation (if applicable)
  • Medical Power of Attorney or guardianship papers (if applicable)

Step 3 - Review and Admission

Patient information will be promptly reviewed with the psychiatrist and treatment team. Our goal is to have a decision and direction for you within 1 hour. Medical clearance may be required before admission.

The patient will be directed to Archbold Grady at 1155 5th St. SE Cairo, GA 39828.
